Care plans that promote optimum health and recovery will be developed and updated by our health care professionals. You will receive the information, treatment, and therapies to ensure a rapid recovery, under the supervision of your physician.

Interpreters/translators are available for sensory impaired and limited-English patients at no cost.

 

SKILLED NURSING:

Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) assess your physical condition, communicate regularly with your doctor, and provide the necessary treatment. The nurses will help you and your family deal better with the medical problems you are confronted with, thereby improving your physical and medical condition. The skilled nursing services we provide include:

Catheter Care
Wound Care
Infusion Care
Ostomy Care
Respiratory Care

 

CERTIFIED HOME HEALTH AIDES:

With the supervision of an RN, Home Health Aides will assist you in personal care and the performance of the activities of daily living (ADLs), such as bathing and feeding, until you are able to be independent again.

 

MEDICAL SOCIAL WORKER:

Helps you with utilizing your family, community, and financial resources appropriately to hasten your recovery.

 

PHYSICAL THERAPY:

Assists you to regain strength and coordination for the best (functional mobility possible. Services may include therapeutic exercises, ambulation training, pain management, safety education, and other treatments which are determined by your needs.

 

OCCUPATIONAL THERAPY:

Assists you to regain function and learn compensatory techniques. The treatment goals include helping you achieve your maximum potential in the performance of ADLs.

 

SPEECH THERAPY:

Helps you with your swallowing or speech problems resulting from a debilitating disease or a stroke.

 

 DIETARY CONSULTATION:

Provides individualized counseling to help incorporate your prescribed diet into lifestyle.  

 

INDIVIDUALIZED EDUCATION:

Individualized teaching programs are developed for each patient to help them and their families be well-informed, understand the medical problems, and get back on track.

 

DURABLE MEDICAL EQUIPMENT:

Arrangements for durable medical equipment will be made to meet the needs for the patient.
